Reading and navigation

Page headings, labelled controls and text alternatives help explain the content. If a label, image or document is unclear, tell us which information you need.

Email your accessibility feedback

A different way to get help

If a form or page is preventing an enquiry, email a short description and the format or contact method that works for you. We can discuss an alternative way to help.

How do you report an accessibility barrier?

Email [email protected] or use the contact page. A short, non-sensitive description is enough to start.

Where it happened

Share the affected page and the task you wanted to complete.

What you experienced

Describe the problem and the result you expected. Add device or browser details if helpful.

How we can help

Tell us your preferred reply format or contact method and any approaching deadline.

Do not include passwords, identity documents, tax records or payment details in an accessibility report.

Accessibility statement

Our target and the limits of this statement.

WCAG describes accessibility requirements; a target does not establish full conformance.

Accessibility target and current scope
FormLLC’s accessibility target is WCAG 2.2 Level AA. This statement describes website support and feedback routes; it is not a certification that every page, document or complete service process conforms. A full site-wide conformance assessment has not been established by this statement.
Third-party pages and documents
Payment providers, government websites and other independently operated services may behave differently. Tell us where you encountered the barrier so we can identify the relevant step and discuss available assistance. We cannot change an external provider’s interface or make its decision.

This statement was updated on 22 October 2026. Please report barriers rather than assuming a feature works with every device or assistive technology.

Questions answered

Website accessibility questions

How to contact us and what this statement covers.

How do I report a FormLLC website accessibility problem?

Email [email protected] or use the contact page. Include the affected page, the task you were trying to complete, what went wrong and the reply format you prefer. Keep the first message non-sensitive.

What if I cannot use the contact form?

You can email [email protected] directly. Describe the barrier and how you would prefer us to respond. You do not need to complete the website form to send accessibility feedback.

Does FormLLC claim full WCAG 2.2 AA conformance?

WCAG 2.2 Level AA is the accessibility target. This page does not claim that all pages, documents, authenticated workflows or third-party services have passed a complete conformance assessment.

Can I ask for information in another format?

Yes. Tell us which information you need and the format or contact method that works for you. We can discuss available assistance without promising that every document or external service can be changed.

Should I attach identity, tax or payment documents to a barrier report?

No. A page link and non-sensitive description are usually enough to start. Do not email passwords, identity documents, tax records or payment details as accessibility evidence.

What should I do if the barrier affects an approaching deadline?

Mention the deadline and the step you cannot complete when you contact us. An accessibility report does not itself submit a government filing, extend a deadline or confirm that a payment has been completed.
